Completing the workshop registration by snail-mail

1. Choose the type of workshop you want to attend.
2. Choose the location and dates for the workshop that you want to attend.
3. Get a Workshop Registration Form from your sponsor or print one by clicking Workshop Registration Form.
4. Complete all information on the Workshop Registration Form (including the Member's information of the Member that inspired you to take the workshop or for a referral contact Creative Questions's Office by calling (800) 873 - 5828 or by emailing Office@CreativeQuestions.com.
5. Sign it.
6. Add the payment.
7. Mail it to the address at the bottom of the form.

Now go back to step 1 and follow through to register attendance.

When your Workshop Registration Form is received and accepted you will receive a Welcome Letter and a Workbook (if a Workbook is available for this type of workshop and if your registration is received more than two weeks before the start of the workshop) in the mail.
